In this episode, a man named Jason (played by Evan Peters) takes Dr. Gregory House, Dr. Remy "Thirteen" Hadley, and several patients hostage in Cuddy's office. Frustrated by years of undiagnosed chronic pain and fatigue, he demands a correct diagnosis from the "best doctor" or he will kill the hostages.

Jason demands that Thirteen test the experimental medications on herself first to ensure they aren't poisoned or traps. This puts her life at risk, particularly due to her existing Huntington’s disease.

An ambient track from the album "The Dead Texan" (specifically the track " The 6-357 ") is used during the final scene where House and the patient share a knowing nod.

House eventually diagnoses Jason with melioidosis , a rare tropical infection. The episode ends with Jason realizing he can breathe deeply again after the treatment works.
